Shoulder pain pumps are sold under the brand names Stryker Pain Pump and I-Flow On-Q Pump. They are often prescribed for those who have undergone arthroscopic shoulder surgery. During the surgery a small pump is implanted in the joint and is intended to stay in place for several days after surgery to deliver pain relieving medication.

However, recent studies have indicated that these pumps may deliver too much medication, and that the result may be joint damage resulting in a painful condition known as PAGCL (Postarthroscopic Glenohumeral Chondrolysis.

Unfortunately, the symptoms of PAGCL do not show up until 3-12 months following surgery. These symptoms may be attributed to other causes and the connection to the pain pump may be overlooked. They include:

  • Increased pain in the shoulder, both when the joint is in motion and at rest
  • Increased shoulder stiffness
  • A clicking, grinding, or popping of the shoulder joint (known as crepitus)
  • Decrease in range of motion of the shoulder joint
  • Loss of shoulder joint strength

These problems are symptomatic of cartilage deterioration. Damage resulting from PAGCL is permanent and is either irreversible or requires extensive surgery. The July issue of the American Journal of sports Medicine published a study of 177 shoulders, 19 of which had arthroscopic surgery with the implant of a pain pump. Of those 19 surgery patients, 12 developed PAGCL. This is an alarming rate. Shoulder pain pumps are currently on the FDA’s medical watch list.
